Parents of students participating in athletics at Putnam County R-I schools for the 2019-2020 school year: mark your calendars now for May 20 - June 14, 2019.

Putnam County Memorial Hospital Rural Health Clinic is providing FREE sports physicals for students from May 20-June 14, 2019.

Students must bring necessary forms with them and a parent/guardian must be present. Pre-Participation Physical Forms
