QUALIFICATIONS
The Putnam County R-1 High School (PCHS) is soliciting nominations for consideration as members of the Unionville High School/Putnam County R-1 High School Academic Hall of Fame. The dual purpose of this recognition is described as follows:
1. to honor alumni who have achieved extraordinary success in high school and college academics
and in their professional careers and
2. to provide role models to current students of Putnam County R-1 High School.
To be considered for this honor, the following criteria must be fulfilled by the nominee:
• entire high school career was at UHS and/or PCHS
• graduated from UHS or PCHS
• graduated twenty (20) years or more prior to the date of nomination
• at least two letters of recommendation (at least one of those must be from a professional peer)
• completed application signed by the person making the nomination and submitted by May 31.
• submitted curriculum vitae or resume of academic and professional history of the nominee
• nominee must be living at the time of nomination

Nominations may be made by self-nomination.
CV or resume for individuals nominated and not selected for induction in a given year will be filed
and reviewed each succeeding year.
A standing committee* will select the next class of honorees who will then be honored by the
hanging of a plaque that includes the honoree’s name(s), accomplishments, and photo(s) in the
PCHS commons. Honorees will be posted on putnamcountyr1.net and our social media channels.
*A standing committee member who may be related in any way to a nominee will be excused.
